As of August 2025, tile purchases have resumed. You may now place your remembrance tile order following the instructions below! Please remember that tiles purchased before April 1 will be unveiled at our annual Memorial Day Ceremony, while tiles purchased before September 1 will be unveiled at our annual Veterans Day Ceremony.
To make a Remembrance Tile purchase, please follow these steps:
- Click 'Download PDF' below. The PDF file below is an e-copy of the tile order form.
- Once you have downloaded the PDF, please print out a physical copy of the tile order form.
- On your physical copy, please write-in the name, position/ranking/branch, and any other information you desire based upon the allotted amount of spaces available per the purchase option you are moving forward with. Please do not enter more than one character in a box.
- Once you have your tile order form filled out, please seal it in a stamped envelope along with your payment (check or cash), and mail the envelope to 838 W 13th St, Upland, CA 91786. You may write it to the attention of Upland Veterans Monument.
- Please ensure that both the tile order form and your payment are in the same envelope and that all of your contact information is filled out on the tile order form.
- If writing a check, please make the check payable to the Upland Enhancement Foundation. Its' tax ID is #92-2299235. In the memo line, please write "Upland Veterans Monument".
- You will be contacted upon receipt of your order and we will assist you regarding next steps as needed. We will also inform you of the tile unveiling date upon receipt of your order.
