How to buy a tile

We have three different size options for our remembrance tiles. Sizes available include 4x12, 8x12, and 12x12 (sizes are in inches). Please see the two-page PDF file below for more information and pricing.

Steps on how to purchase remembrance tiles on the cubes in the plaza:

1. Please click on the "Download PDF" button below. After downloading, proceed to print out the order form.
2. After printing out the order form, fill out the lines of text according to the size option you desire. Please make sure to write in one (1) letter per box. Please be sure to also fill out your name, address, phone number, and email address in the designated spaces at the top right.
3. Make checks payable to the Upland Enhancement Foundation 501(c)3. Tax ID #92-2299235. Please write-in "Upland Veterans Monument" for the memo line.
4. Mail in your completed order form and your check to the following address:
